NHS Sunderland CCG wish to commission a Multi-specialty Community Provider (MCP) for the delivery of a range of community-based health and care services for the registered patient population covered by the CCG. It is anticipated that the MCP will, in summary:— Have a contract term of up to 10 years, based on 2017/18 budgets is expected to be between approximately 209 000 000 GBP and 240 000 000 GBP;— Manage a single integrated budget for out of hospital care;— Transform access to and delivery of community health and care services; and— Meet a defined set of outcome and performance measures.
