NHS Bradford Districts CCG Hypertension/Healthy Hearts

Hypertension is an independent risk factor for CVD. Controlling blood pressure in a big population is challenging and done the usual way consumes a lot of resources. The recently published TASMIN SR study (McManus et al 2014) suggests a new way to manage blood pressure by self-monitoring of blood pressure (SMBP) and self-titration (medications dose adjustment) done by patients. A mean difference in blood pressure was 9.2 mmHg between the intervention and control groups. This significantly reduces risk of Stroke and heart attack/ Myocardial infarction (MI). Also this model is sustainable as it helps patients develop knowledge and skills to SMBP and remove burden from GP practices. In the past year approximately 3000 patients were diagnosed with hypertension in Bradford and the mean blood pressure in the entire population of patients with hypertension was approximately 130 mmHg. If a reduction of mean BP similar to that seen in the study could be observed within this group of patient then this potentially could prevent significant numbers of CVD events. The information below describes a service that will implement a new model of BP management amongst Bradford Districts CCG practices. This programme will be deployed as part of Bradford Healthy Hearts (BHH) programme. http://www.bradfordshealthyhearts.co.uk BHH is a programme that aims to reduce numbers of strokes and heart attacks by 10% by 2020. This intervention will focus on patients recently diagnosed with hypertension. All provisions of this service will be expected to meet and comply with NHS standards for IT Governance, Data Protection, security of general and personal information, data and records held and processed and transmitted (via and over all mediums). The successful provider will be required to sign the NHS Standard Short Form Contract for the service and accept all terms and conditions.
