This notice is placed as a Prior Information Notice (PIN) to inform the market of NHS Devon Integrated Care Board's (the Commissioner's), intention to recommission a Care Co-ordination Hub for Devon . The Commissioner would like to communicate planned market engagement event(s) and request expressions of interest for these.The Care Co-ordination Hub provides a senior level, multi-disciplinary, clinical consultation service, including the ability to prescribe; which assesses patients' needs and navigates them to the right service, first time.This exercise is being carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioner.
