NHS Hull Clinical Commissioning Group is seeking to commission a broad range of Community Services through an open procurement process. The CCG wishes to establish a single Lead Provider contract for each of three distinct lots, also termed Care Groups. Care Groups are groups of services which the commissioner believes are naturally aligned and based around specific client needs/integrated pathways/service coherency. The three lots are: • Lot 1 - Integrated Sexual Health Service • Lot 2 - Urgent Care; and • Lot 3 - Integrated Community Care Service Lead Providers will be expected to demonstrate how they will deliver outstanding quality services in an environment of transforming health and social care provision as outlined in the Five Year Forward View, the commissioner’s strategy and Hull 2020 programme. Our aim is to achieve transformational change, including rapid response; care delivered at home as the default setting; seven day working, with 24/7 care available according to need. To this end, Lead Provider tender submissions will need to incorporate the following principles of service delivery: Care coordinated around the needs of the patient: The Provider must deliver a model of care coordination across all services in the Care Group. As a minimum, the model must include multi-agency collaboration with one individual responsible for coordinating and navigating the patient’s journey through the health and social care system, acting as the coordinating point for all of their care. Care coordination needs to be available to all patients in the pathway, particularly those who are frail and/or have chronic and complex conditions and must include provision for the patient’s carers and family. Holistic care planning and assessment: The Provider must ensure services adopt a holistic based approach to care with clinical assessment looking more broadly across the patient’s needs. Where appropriate, teams across the Care Groups will need to include specialist staff although there is a need for multi-skilled generic workers who can bridge the gap between health and social care and take responsibility for looking after the whole person rather than performing individual tasks. Integrated, multi-disciplinary teams wrapped around primary care: In delivering services in the Care Group, the Provider must ensure that teams of community staff are developed around groups of practices and forge very close relationships with them. Social Care: The Provider must ensure that the delivery of services in the Care Group is integrated with the delivery of social care provision. The service commencement date is 1 April 2016 for a duration of 5 years with the option to extend for a further 2 years.
