Descriptions
The neuro-navigator service will work collaboratively with local stakeholders to share learnings and provide education regarding the services available for neuro-rehabilitation patients to ensure that the system is maximising on the local services available. The neuro-navigator will be proactive in recommending opportunities for service development by identifying gaps or capacity issues within the system and picking up local intelligence.The aim of the neuro-navigator service is to improve equity of access, experience and health outcomes for patients within neuro-rehabilitation services, across healthcare organisations and geographical boundaries.
