Lincolnshire East Clinical Commissioning Group (LECCG) have commissioned a Nurse Lead Palliative Care Bed Service in the Boston locality of Lincolnshire for people requiring End of Life Care.This is an in-patient generalist palliative care service, this means that palliative care is the person-centred attention to physical symptoms, psychological, social and spiritual distress and cultural needs in patients with limited prognosis, in order to optimise the quality of life for the Service Users and their families and friends. Generalist palliative care is palliative care that should be provided by all health and social care professionals, within primary and/or secondary care, within their core duties to patients with life limiting illness.This service is dependent on the Provider of the palliative care beds working in partnership with other end of life care Providers including primary and secondary care services, specialist palliative care services, social care services and the third sector. The purpose of this partnership working is to jointly deliver integrated care throughout the end of life care pathway, using recommended best practice end of life care tools, including but not limited to the Gold Standards Framework, the Liverpool Care Pathway (or its replacement model), the Preferred Priorities of Care and Advanced Care Planning in order to promote choice and improve care for Service Users in the last year(s) of life.The Nurse Led Palliative Care Beds in Boston Contract has been awarded to Lincolnshire Community Health Services NHS Trust.
