The outreach nurse conducts health assessments, including assertive street outreach, supports individuals in accessing onward care, and is embedded within the wider rough sleeping multi-disciplinary team (MDT). This role addresses barriers to healthcare for homeless individuals, working to increase access and reduce emergency admissions.
