Descriptions
The objectives of the Leeds Intermediate Care Remodelling programme are to:, - Implement a single integrated model of intermediate care that embraces bed-based, homebased intermediate care and re-ablement, - Shift the focus of intermediate care from a bed-based response to a care at home response, - Regain increased focus on admission avoidance as well as facilitating hospital transfers of care, - Reduce the intermediate care length of service through focussed intensive therapy approaches, - Simplify the pathway into and through the new integrated service model to provide the best possible experience for people and for professionals, - Simplify the pathway out of the new integrated service to ensure that people have the right ongoing care, support and connection to their communities, - Achieve the best outcomes for people within available resources., The intention is to establish a partnership of interested stakeholders to, commence the co-design of a new model through a Programme Board and events for stakeholders. The design and implementation of the new model will take place during 2022/23 with a view to the new model being operational from April 2023.
