Primary Care Information and Facilitation aims to provide a comprehensive primary care clinical information and facilitation service for NHS City and Hackney Clinical Commissioning Group in conjunction with all GP Practices to improve clinical outcomes for patients in primary care. The service provides detailed support including on-site teaching and facilitation programmes to improve quality across practices uptake of clinical guidelines, design and facilitation of templates for clinical systems and decision support tools. Supporting practices with implementation and audits. The service will provide dashboards, statistical analysis and interpretation of clinical data to meet the requirements of NHS City and Hackney Clinical Commissioning Group as commissioners., The Referral Form Management aims to centralise referral form management across health and social care services in City and Hackney in order to improve consistency, quality, efficiency and effectiveness of referrals.
