PRJ-882 Home from Hospital Supported Discharge Service

Barnet CCG would like to engage with potential providers with relevant expertise and experience to deliver a Home from Hospital Supported Discharge service. It is envisaged that the service will include supporting around 130 vulnerable patients to be discharged from Barnet and the Royal Free Hospitals and Edgware and Finchley Memorial Community Hospitals a month. Patients will be offered practical and emotional support in their homes and place of residence. Support will be of a one off nature for some patients and up to 4-6 weeks for others, depending on need.Social prescribing will form a key element of this service, and patients will be supported to connect with local voluntary and community services to help improve their wellbeing and social welfare
