Islington CCG’s Integrated Care programme is working to improve the coordination of care for adults with complex health or care needs. This involves more collaborative working between key stakeholders with GPs, health and social care professionals and non-traditional provider (NTP) organisations coming together to organise person centred care more effectively. We also have a particular focus on supporting people to access support in the community in order to help them define and meet their goals. We are seeking to build on the experience of delivering a navigation service over the course of the last 5 years and support the development of Islington’s personalisation offer. The value of the contract over 41 month contract period and the possible 24 month extension is £2,047,500 inclusive of VAT.
