The NHS Wales Shared Services Partnership, hosted by Velindre NHS Trust are acting on behalf of Cardiff and Vale University Local Health Board who are seeking to establish a contract for the provision of a Community Assessment and Reablement Unit.The unit is aimed at providing temporary accommodation and care for individuals whose discharge destination is their own home (ie not care home setting), who have been assessed, treated and stabilised in hospital and are now medically fit for discharge but require convalescence/a period of reablement or confirmation/completion of discharge plans. There is a target length of stay will be less than 14 days.The main principles underpinning the CARU are:- To provide an appropriate environment in which to meet the individual’s abilities, needs and requirements for support.- Collaborative working across all partner agencies to maximize the opportunity to deliver seamless and individually tailored care, with health improvement interventions embedded within the approach- Provide holistic, evidence-based, client centred care- To work with the individual and their family to maximise his/her potential to regain independence so as to be able to return to their usual place of residence/ability to make informed choices regarding ongoing support and care.NOTE: The authority is using eTenderwales to carry out this procurement process.
