Provision of a Day and Evening Opportunities Service for People with Disabilities

Torfaen County Borough Council is seeking a partner to take forward the evolution from traditional day activities provision to a service which will support people with disabilities to access the day and evening opportunities that they want. The Council requires a service which can both support service users to achieve their potential through direct provision and facilitate a change in the way people access day and evening opportunities either with support, with peers or by themselves.The Council is not looking to procure a traditional day service. It is looking for innovation. The successful tenderer will be able to demonstrate how it can build a service which can deliver the following outcomes:*People with disabilities in Torfaen are treated as an individual through person centred approaches *People with disabilities in Torfaen are socially integrated through their daytime and evening opportunities*People with disabilities in Torfaen are perceived more positively in employment, learning and accessing the community*People with disabilities in Torfaen achieve their educational or learning potential *People with disabilities in Torfaen achieve their employment or volunteering potential*People with disabilities in Torfaen develop skills / interests / hobbies*People with disabilities in Torfaen develop peer networks and support*People with disabilities in Torfaen develop safe, stable and supportive relationships*People with disabilities in Torfaen develop Life Skills and Independent Living Skills*People with disabilities in Torfaen actively participate in planning for their own futures and make informed choices*Carers, family members and other representatives are given relevant information where appropriate*Carers, family members, friends and the wider community are involved in supporting people with disabilities in achieving their potential*Disabled Children aged 16+ in Torfaen are supported and encouraged to make a positive transition into adulthood The contract will be for an initial period of 3 years with an option to extend for a further 2 years subject to a successful evaluation and/or developing collaborative opportunities.There will be a maximum budget of £200,000 per year to purchase various levels of support to meet the outcomes above. The tender price will be scored and must be affordable, but the emphasis will be on the quality of the proposed service in meeting the outcomes in the service specification.
