Purpose of the Service: Provision of a Learning Set for organisations working in Wrexham, Flintshire, Denbighshire, Conwy, Gwynedd and Ynys Mon, supporting children and young people with disabilities (0-25) and their families and other families with children and young people affected by disability.Term:The service will run for 1 year from 1 May 2018 until 30 April 2019.
