PROGRESSION SERVICE : INTERMEDIATE CARE FUND 2016/17Wrexham County Borough Council and Betsi Cadwaladr University Health Board are in receipt of Welsh Government Intermediate Care Fund (ICF) funding (Tranche 1 monies, for people with a learning disability and/ or children with complex needs), and are looking for tender applications from organisations that are able to meet the required service aims and objectives as detailed below:The Provider will work with providers of care and support within a targeted number of Supported Living Houses in order to provide short-term intensive support to enable the Provider and service user to co-produce and implement a Progression Plan, which will enable the service user to achieve greater levels of independence and confidence, develop community and personal network, as well as reduce reliance on paid services/ support.The Provider will work closely with Health colleagues, especially in the field of Therapies.The Provider will be familiar with working with a range of outcome measures and have experience of working with service users and providers of care and support to monitor enablement.The aims and objectives of the service are:¿To reduce reliance on paid support / services through developing skills and independence¿To assess individuals skills and develop a programme to progress their level of independence¿To put in place individual Progression Plans, which include outcomes which focus on progression and independence, and which include, where appropriate, close working relationships with Betsi Cadwaladr University Health Board’s Progression Staff to deliver a holistic response to need.¿To develop a system to monitor progression¿To support people to develop the skills to improve their opportunities to access employment and/ or educational opportunities¿To support people to manage their health and well-being effectively¿Connect with local community networks and third sector servicesThe service will run from 1 March 2017 to 31 March 2018.NOTE: The authority is using eTenderwales to carry out this procurement process.
