Descriptions
This is a holistic healthier lives service aimed at reducing health inequalities and improving health and wellbeing. It directly links to the Staffordshire County Council (SCC) contact centre (using a single point of access approach) and is designed to target Clients with one or more lifestyle risk factor, requiring additional help to change their behaviour.At its core, the Healthy Lifestyles Service will provide behaviour change support around:— stopping smoking,— reducing alcohol intake to a safe level,— weight management (for children and adults).This service will:— assess and prioritise needs and risks,— assess readiness to change,— provide Clients (as appropriate) with an overall lifestyle support package which includes the development, management and review of behaviour change plans,— offer a range of targeted behaviour change interventions that are appropriate to the Client and the range of needs they present with,— facilitate Clients and families access to appropriate information, advice and guidance and self-help tools,— facilitate Clients and families access to access appropriate community prevention programmes and to other Services that influence wider determinants of health or Services providing specialist support beyond the scope of the Healthy Lifestyles Service (as appropriate),— work effectively as part of a consortium / collaboration / joint venture / partnership to ensure individuals, families and communities experience a seamless pathway, regardless of multiple / complex / changing levels of need.It will also:— be inclusive and treat each Client individually, providing a personally tailored and integrated support package to individuals and families, focussed on a whole person / whole family approach and the range of needs that they present with,— be innovative in its approach to ensure that is both accessible and perceived to be of a high quality to both commissioners and service users and improve the health and wellbeing of its users.
