Provision of Learning Disability Services: Accommodation Based Services Including Housing Support, Care at Home and Community Based Activities/Day Support. The aim of the Service shall be to provide a safe and supported environment to meet the needs of adults (18 to 65 years old) with varying complexity of support needs and intensive health needs and conditions to live as independently as possible, by mitigating the impact of their mental health problem through support with daily functioning and maintaining their recovery.
