Call off from Crown Commercial Services G-Cloud 13 Framework. In line with the steer from NHS England, the commissioner is seeking to offer patient choice of provision to support improved uptake of structured education, by commissioning a remote, digitally-enabled structured education service, Oviva Diabetes Support. The aims and objectives of service are: - Provide Oviva Diabetes Support to people with type 2 diabetes registered with a GP practice in Buckinghamshire CCG as an alternative to face-toface education. - Support participants to increase confidence in self-managing their condition. - Support participants to make lifestyle changes that offer benefits for their long-term health, including improving HbA1c and weight loss, aligned to personal goals. - Support reductions in diabetes and blood pressure medication use. - Support reductions in use of primary and secondary care diabetes services. - Support effective behaviour change through the use of evidence based behaviour change approaches in coaching, educational content and digital technology.
