In accordance with the next steps of the forward view, integrated urgent care service specification and the long term plan, the out of hours home visiting service will treat and care for people with an urgent care need in their own home, protecting them from avoidable harm, reducing the number of ambulance call outs and improve patient satisfaction by supporting them at home to manage complex healthcare needs, including end of life care, thereby reducing the need for hospital admission where it’s clinically appropriate to do so.This service will provide urgent home visits when GP surgeries are typically closed, from 6:30 p.m. to 8:00 a.m. weekdays, all day at weekends and bank holidays.
