Descriptions

South East Staffordshire and Seisdon Peninsula CCG commission health services for around 210 000 residents and have commissioned a provider to deliver a Psychological Therapies Service (Adults) to South East Staffordshire — covering the population of Burntwood, Tamworth and Lichfield (161 000). The service shall provide a stepped care model with people receiving care and therapy relevant to their needs; including recommended treatments according to NICE guidelines for anxiety and depression, but in addition services to support functionality and social well-being. The expectation is that individuals will be offered the least intensive intervention that is likely to result in clinical improvement. The model shall be delivered by a team attached to a general practice; thus providing each practice with an identified therapist/counsellor who will provide a contact point to the service, provide advice, guidance and support to the practice. Psychological Therapies Service (Adults) will aim to provide a self-accessed; evidence based psychological therapy service for the local population, to increase wellbeing and mental health resilience and to increase community assets. The objectives of the procurement are: — Provide choice of therapies dependant on patient's needs — Provide easy, timely access to suit patients ( i.e. hours of service, weekends, local location and facilities) — Improve the condition for which the service user was referred. — Provide tailored individual care plans describing anticipated treatment and discharge planning — Ensure the integration of the service user into the community. — Provide signposting, information and support to facilitate access to a range of community based support services — Improve identification and awareness of common mental health disorders (e.g. through awareness training for a range of health, social care, education and welfare professionals and voluntary sector) — Improve emotional wellbeing, quality of life and functional ability in people with common mental health disorders. — Improve individual's well-being and functionality, this will include people with physical health problems — Improve access and support to maintain people in work, help them to return to work, help them into education or training and where appropriate help people to find meaningful activity including volunteering and community support — Provide a service that is integrated with GP practices — Provide good patient experience/satisfaction The service will contribute to: — A reduction in exacerbations of severe psychological conditions — Building mental health resilience in the community — The well-being of people with long term conditions — Users feeling able to contribute towards society South East Staffordshire and Seisdon Peninsula CCG have a target to treat 15 % of the population suffering from anxiety and depression (2061).
