Rapid Response and Resettlement Transport Service for Older Patients
Descriptions
NHSGG&C and HSCP require a service to transport older patients returning home from assessment units, assessment wards, discharge lounges, designated wards and ED’s at the QEUH and Glasgow Royal Infirmary and resettling them in their home and providing support which may include but not limited to getting provisions, making refreshments and securing property on leaving. The service also supports the intermediate care services which involves taking older patients from acute sites to step up units and taking older patients from step down beds in nursing homes in their own homes or with an alternative care provider. This business sector is at risk of infiltration by Serious and Organised Crime (SOC). NHSGG&C has therefore enhanced its criteria to participate and has implemented its SOC mitigation protocol.
