Slough Borough Council invites suitably qualified and experienced organisations to provide Outcome Focussed Care and Support for Adult Social Care Service Users living at home. The tender will be divided into two Lots but the key aim for both services is to help people live fulfilling and independent lives, ensuring access to care and support that meets individual needs, working with the local community to achieve better outcomes for individuals.Lot 1 Outcome Focussed Care and Support within the Home.Providing packages of care and support designed to meet individual circumstances. Service provision will be provided in the Service Users own home and must be flexible and responsive ensuring that individuals are able to exercise choice and control over the services they receive. It is envisaged that between 15 and 20 suppliers will be appointed. Estimated value of 12 800 000 GBP. No award was made for Lot 1.Lot 2 Outcome Focussed Care and Support within Extra Care Housing.Providing services as described in Lot 1 to Adult Social Care Service Users within Extra Care Housing across two sites within the Borough. It is envisaged that between 1 and 3 suppliers will be appointed. Estimated value 2 434 000 GBP.
