Self-Care Participation and Engagement Programme for NHS Newcastle Gateshead CCG

Background: Newcastle Gateshead CCG has been successful in becoming a Vanguard site for Enhanced Care in Care Homes. A number of work streams have been identified: •Pathway of Care - within the new model of care we are developing a pathway of care for older people, and we aim to improve care by focussing on key areas, these are Responsive Care, Primary Care, Nutrition and Hydration, dementia care, Palliative Care and End of Life Care, Telehealth/Telecare/Telemedicine, medicine management. •Outcomes Workstream - the aim of this workstream is the development of clearly identified outcomes for the New Pathway of Care. This will ensure the patients voice is at the forefront of developing outcomes. •Workforce Workstream - as part of this workstream we aim to develop a Workforce / Education and Training Strategy. The strategy will identify the skills of existing workforce, identify any gaps, look at cultural shifts and look at the training and support needed to achieve our outcomes. •Commissioning, Contractual and Payments Model Workstream - as part of this work stream we will develop an appropriate commissioning, contractual and payments model that will suppo Aims and Objectives As part of the Involvement and Engagement work stream, we are developing a Self-Care Participation and Engagement Programme. The aims and objectives of this programme are outlined below: •Support volunteers by increasing their skills, knowledge and confidence to engage with communities •Educate volunteers to manage their own health and care •Encourage other patients and members of the public to become involved in the Vanguard Care Home Programme •Create a social movement by incorporating a ‘train the trainer’ element to the programme to ensure long term sustainability What’s required: •Programme to be developed and co-designed with equal participants who are aged 65 plus, receive care, are carers themselves or may require care in the next 5 to 10 years •Programme to be advertised with assistance from Newcastle Gateshead CCG •Deliver programme to 1 cohort (15-20 people). Timescales for this programme should be directed by volunteers and Newcastle Gateshead CCG •A few of events which will include the ‘train the trainer’ element and provide sustainability to the programme – we would aim to deliver these events to a larger group of participants than cohort 1 •Programme success is evaluated once the delivery is complete and six months after to measure impact of volunteers who have participated in the train the trainer element of the programme. •Provide details of how you will internally evaluate •Links to the independent external evaluation of the whole programme •Provide details on how this programme will be sustained in the long term?
