Denbighshire Community Support Services are testing the market to see if there are organisations with the relevant skills Knowledge and experience to deliver a service to support Carers of citizens aged 65 years and over with a diagnosis of dementia.The service will be for a period of 12 months, commencing 1st April 2018 to 31st March 2019 with the amount of funding available being £31,000.AIMS AND OBJECTIVES OF SERVICE1To provide information, advice and support to Carers of citizens aged 65 years and over with a diagnosis of dementia, which will help the individual with dementia to remain in their home for as long as possible.2To provide a service that focuses on ‘what matters’ to the Carer, and supports the Carer to achieve his/her own well-being outcomes.3The service is intended to be proactive and an alternative to a ‘crisis management’ approach.4To provide Carers with a better understanding of dementia, and with specialist information and support that is separate from statutory provision.5To support Carers to become involved in the planning of future dementia services in Denbighshire.6The provider must have a good understanding of the requirements of the Social Services & Well Being (Wales) Act 2014. It must also embrace the ethos of the Act, and Denbighshire’s approach to meeting the requirements of the Act.SERVICE OUTPUTS1 The services that will be provided for this Agreement are:a)Identification and provision of support to new and hidden Carers.b)One to one support for Carers, in particular Carers isolated by their caring role.c)Prevention of deterioration of the caring situationd)Support Carers to achieve their own well-being outcomes thereby enabling Carers to continue caring at home, and live a fulfilled life despite their caring role.e)Improved knowledge and understanding of dementia, and confidence for Carers to support them in their caring role.2 In regard to the target group of Carers, the provider will more specifically:a)Work jointly with specialist EMH teams to ensure that Carers are referred for support.b)Ensure that Carers receive timely, up to date information that is relevant and appropriate for Carers caring for persons with organic mental health conditionsc)Provide support for Carers, and ensure that equitable services are offered across the County which are accessible to all Carers of individuals with dementiad)Promote the social inclusion of Carers, and provide opportunities for mutual support amongst Carerse)Facilitate and support Carers to be involved in the strategic planning of services, in particular the development of a Dementia Strategy and a Dementia Pathway for Denbighshire.NOTE: To register your interest in this notice and obtain any additional information please visit the Sell2Wales Web Site at http://www.sell2wales.gov.wales/Search/Search_Switch.aspx?ID=75964.
