Sharing Lives Service – Spot Purchase – Any Willing Provider

Descriptions

This is a 1 year “Willing Provider” framework contract (with a possible 1 year extension option) to provide Torbay and Southern Devon Health and Care NHS Trust a Sharing Lives Service (previously known as Adult Placement and Shared Lives). The Service will offer to people aged 18 years or over a highly flexible form of accommodation and with care and/or support, provided by individuals or families in the local community and will deliver agreed elements of eligible need as agreed following a social care assessment.
