The Short Term Reablement service is provided in registered residential care homes. The service provides reablement, designed to help individuals to regain skills and independence following a hospital stay. It can also be used to prevent hospital admission. The service is designed to maximise independence of older people over a period of approximately six weeks to support them to regain independence lost following an acute episode or other incident.
