SLAM is seeking to purchase a Caseload Weighting Management Tool / Application with algorithm for predictive analysis functionality (functional or in development) which would enable frontline staff, care coordinators and care teams, plan and prioritise care and resources at a local level based on needs and risk of the patient.The tool should be able to enable: • Support, through supervision to ensure flow through the Community pathway.• Provide drill down facilities from the dashboard view• Provide Patient Access level data i.e. understanding what the key triggers are and other issues surrounding the patient• Outward referrals capabilities internal and external• RAG patients based on needs and riskPlease complete the SLAM Caseload Mgmt tool - Supplier Soft Market Test template v1.1 and submit this via Delta
