NHS 24 wish to identify partners who will work with them in a collaborative manner to developLot 1. Digital diary/calendar - to support more effective care planning and coordination for the person and their career.To support the person with the co-ordination of their care plan a diary/calendar will be developed which will integrate the persons' daily activities and support an independent lifestyle.Lot 2. Person held file – this may include a web based personal site which will enable the person and their carer to share information about themselves to health and social care professionals, support workers and anyone they deem important to their support network. This site will facilitate the persons' access to tools/apps to promote their health and wellbeing and help them achieve the outcomes they have prioritised.This information can be shared with other professionals and key supports and can also be used in emergency situations.Lot 3. Digital tools for independence and enablement - The site will assist professionals to design programmes of intervention with the person, which match their identified outcomes. We want to develop an electronic assessment tool, which will support all stages of the assessment process including self -assessment. Where a person's needs are complex, the tool will assist professionals to make consistent use of best practice and knowledge in the area of falls.The technology to be procured will be web-based applications and apps for mobile devices that are designed for use by the general public on consumer devices such as PC, laptop, tablets and mobile phones.Services will include requirements analysis and design of applications by engaging with end users, development and support of the applications and the IT infrastructure.Functionality will broadly cover search and presentation of content about the local area, various tools (eg. personal diary) and games, and managing a personal record.Applications will need to securely manage user accounts and sensitive personal information.Integration with statutory systems in Health and Social Care is expected. Similarly, integration may be required with third and private sector service providers.There is also a need to support service providers with the ability to analyse data.SmartCare users will provide valuable information which we will want to interrogate and use to design future services.Additional applications for professionals may be required in order to support the assessment process.
