Descriptions
The service will deliver the stage 1 model of support, providing a minimum of 25 units of accommodation with intensive support to those who are homeless or imminently at risk of homelessness. Staff must be available on site 24 hours a day 7 days a week.The Services will deliver holistic support that enables Service Users to manage crisis, avoid eviction and repeat homelessness, facilitates engagement with other services as required and reduces the likelihood of crises happening in the future. Service Providers will be required to target services and provide personalised support to meet a wide range of needs. Service Providers will proactively manage expectations of Service Users whilst supporting them to achieve health, wellbeing and wider social inclusion outcomes. The Services to be delivered by the Service Provider will act as a catalyst for improving inter-agency approaches and partnership working to meet the needs of people who are or who are at risk of becoming homeless. The Services will provide flexible and responsive support to vulnerable people who are in need of support because they are homeless or at risk of becoming homeless. This may include but is not limited to, people with alcohol and substance misuse issues, mental health problems, personality disorders, physical disabilities or long-term physical health conditions, learning disabilities and those with a history of offending behaviours.
