NHS Wandsworth CCG is looking to appoint a suitably qualified and experienced provider to co-ordinate and deliver a social prescribing service across the Borough.The social prescribing Service will:— improve social and clinical outcomes for people with long-term conditions and their carers,— make more cost-effective use of NHS and social care resources,— enable clinicians to refer patients to a range of local, non-clinical services to improve their health and well-being,— address people's needs in a holistic way and facilitate access to the right place, first time,— contribute to building a more supportive, inclusive and resilient community.
