South East Staffordshire & Seisdon Peninsula CCG sought Formal Expressions of Interest for the provision of a Pulmonary Rehabilitation Service as a cost effective intervention for the management of lung disease.Pulmonary Rehabilitation has established itself as a key management strategy for Patients with chronic respiratory disease such as Chronic Obstructive Pulmonary Disease (COPD). The National Institute for Health and Care Excellence (NICE) recommends that pulmonary rehabilitation should be made available to all appropriate Patients with COPD including those who have had a recent hospitalisation for an acute exacerbation.The Pulmonary Rehabilitation service aims to be an equitable, patient centred service that and shall offer the best possible outcomes for patients to improve their quality of life and reduce their symptoms. The multi-disciplinary service will offer eligible patients across South East Staffordshire with access to a programme of exercise, education and anxiety management.The service shall be integrated with a COPD clinical pathway and enable clinicians from Primary and Secondary care to refer patients to the service, offering patients choice and flexibility to attend a programme.The key objectives of the Procurement were:— To provide timely access to Community Pulmonary Rehabilitation.— To improve patient confidence in self-management of chronic respiratory diseases.— To fast track patients who have been discharged from hospital within one month following exacerbation to prevent readmissions.— Improve health related quality of life for COPD patients.— Implement a cost effective intervention for COPD patients. Provide good patient experience/satisfaction.The service shall contribute to:— Reduction in the number of admissions and re-admissions for COPD.— Reduction in mortality rates for COPD.— Reduction in exacerbations for COPD.South East Staffordshire – covers the population of Burntwood, Tamworth Lichfield within South East Staffordshire.There are currently 2,663 patients with a confirmed diagnosis of COPD in NHS South East Staffordshire and Seisdon CCG (South East Staffordshire locality only) (2011/12). It is estimated that 450 patients will be eligible for the programme per year.
