Lot 1: Integrated Community Equipment Services (ICES).Kent County Council, in partnership with NHS Commissioning Groups in Kent, is seeking to establish a more singular Integrated Community Equipment Service (please note this does not include the Medway Council and Medway Clinical Commissioning Group area). The Integrated Community Equipment Service will: Procure, store, deliver, demonstrate, install, de-install, maintain, refurbish, collect and recycle equipment for daily living, medical loan equipment, rehabilitation, mobility and specialist equipment. The service is available for citizens of Kent, of all ages, with health needs, physical and sensory impairments. The Integrated Community Equipment Service plays a crucial role in helping the most vulnerable people in Kent remain in their own home. Through the provision of equipment, people are enabled to carry out everyday activities, maximise their independence, or to be supported to be cared for at home. Equipment can reduce the likelihood of hospital admission and can assist in timely discharge from hospital. Children and young people are given the best opportunity to be as independent as possible, including accessing the curriculum in education, and their parents/carers supported to care for them. The effect of this is to increase the opportunity to be educated in a local school and increase educational attainments. Recognising the changing requirements in education, health and social care, the Commissioners are seeking to purchase services more innovatively through a single provider who has experience and expertise across the service range. The Commissioners recognise that, whilst some providers are able to provide the whole range of services, some providers may wish to work in partnership with other organisations. No matter what, the Commissioners expect the future service provider(s) to be able to demonstrate that they have a proven track record in all service areas.Lot 1 will be delivered for the following parties:Kent County Council, Thanet Clinical Commissioning Group, West Kent Clinical Commissioning Group, Swale Clinical Commissioning Group, South Kent Coast Clinical Commissioning Group, Dartford Gravesham and Swanley (DGS) Clinical Commissioning Group, Canterbury and Coastal Clinical Commissioning Group and Ashford Clinical Commissioning GroupLot 2: Digital Care and Assistive Technology (Including Telecare) Services.Kent County Council is seeking to procure a new digital and assistive technology service that will support technology enabled care to be delivered through a variety of different means. Kent County Council has mainstreamed telecare and now has in excess of 5500 users. We have also been trialling other innovative solutions which support people to achieve a range of outcomes. We are seeking to find a lead provider who can provide / source and manage a range of telecare and other personalised digital technology services. Kent County Council provides services to the community to enable independent living. One of the key services is telecare which is used to support people to maintain their independence, facilitating them to remain in their own home for as long as possible and improving their and their carer's quality of life. Kent County Council is looking to source a service to include the following components: 1.Telecare service a. Monitoring — 24 hour service b. Install/de-install and maintenance service 2.Digital Care and Assistive Technology Services. There are a range of existing and emerging digital care services that can deliver better outcomes for people. For example, these could include other types of monitoring services (outside of the mainstream telecare service), web based services to connect people to reduce social isolation, mobile and smartphone based technology, apps and other web based services to support independent and safe living. Future proposals for Digital Care and Assistive Technology (Including Telecare) Services may include telecoaching and telephone assessment and review and, in a fast progressing market the supply and, where required, monitoring of cutting edge assistive technologies. This will enhance the Council's ability to meet the needs of service users to maximise their life opportunities.Lot 2 is delivered on behalf of Kent County Council only.
