Provision of a Tri-borough Steady & Stable Falls prevention service . It is evidence based falls awareness and prevention service to improve balance, reduce falls and enable keep people living independently. It aligns with the 8 week CLCH Falls Preventions Service for people with a higher need and risk of falls. The open referral service offers a 38 week programme to build confidence and maintain independence by promoting an active and healthier lifestyle though physical activity for residents in the Tri-borough. The service outcomes are to improve functioning e.g. posture, balance, mobility, orientation and safety awareness and prevent/reduce unnecessary hospital admissions and other acute care as a result of a fall. The CCG has assessed the most appropriate route to awarding this contract, having regard to the necessity to secure continuity of these essential patient services and has concluded that the incumbent provider will assure the safest solution and being considered the only technically capable provider at this time.
