Descriptions
The aim of the supported discharge from hospital service is to provide people with support following an admission to acute or community hospital or accident and emergency, so they can maximise their independence, self-manage their health and wellbeing and remain connected with their communities. There is a market engagement session booked for Monday 11.12.2017 from 9:30 am to 12:30 pm at IP-CITY, 1 Bath Street, Ipswich, IP2 8SD. With a view to tendering the service in early January 2018.
