Descriptions
A procurement for supported living services will be undertaken for one female Service User with a rapidly progressive form of muscular dystrophy living in her own home in Llandudno. The service requires specialist manual handling procedures, careful positioning and highly intricate moving techniques. The Service Provider will be required to work with Health and Social Services to deliver person centred seamless services in order to meet the Service User's changing health and social care needs. We are looking for a specialist provider with experienced and skilled staff who have good awareness and understanding of muscular dystrophy and who will be required to provide consistent support to promote independent living for as long as possible.
