Supported Living Services for One Service User with Duchene Mascular Dystrophy

Conwy County Borough Council in partnership with the Health Board is planning to commence a tender exercise for the provision of domiciliary care services for one young Service User in his twenties living in his own home within the county of Conwy with a diagnosis of Duchene Mascular Dystrophy. The support required will be 24 hours a day with additional 2 hours per day each day and waking night support. A competent, confident, reliable staff team will be essential to provide continuity and consistency of support. The Provider will be required to be working flexibly and closely with the Health, Social Services and the Service User's family.
