Conwy County Borough Council would like to invite urgent expressions of interest from suitably qualified and experienced Organisations providing supported living services.The Service is required for one gentleman of 35 years of age with a forensic history and challenging behaviour, living in the Conwy area. The Council is looking for a specialist Provider who is able to work as part of a multidisciplanary team.The service required will be 24 hours, waking night with supported installation of a telecare equipment over time.The Provider will be required to support the Service User living independently and safely in the community.
