Primary Care Clusters in Wales were established in 2014/15. There are 14 Clusters within the Betsi Cadwaladr University Health Board (BCUHB) area in North Wales, serving a population of 694,000. The work of each Cluster is led by a Cluster Lead, which is often but not always a GP. Each Cluster is required to identify health and wellbeing needs of the local population and work with partners to respond to those needs, including through the use of Cluster Funding that is made available by the Welsh Government.The South Flintshire Cluster covers the geographical area including Mold, Buckley and villages to Hope on the Wrexham Border. The Cluster includes seven practices, with a registered population of circa 50,000 patients.Primary Care is facing significant challenge that requires alternative approaches to maintain and build further sustainability. South Flintshire Cluster is looking to develop models of working to scale both within primary care and across the primary and secondary care interface.The Cluster are looking to procure the service of a consultancy to facilitate this process.
