This VEAT is to inform the market of the ICB's intention to direct award a contract to Age UK, whilst a procurement process is planned to test the market.The Take Home and Settle Service (THAS) safely support patients' discharge from hospital. Short term practical support is provided in the person's home or other interim accommodation to support independence and prevent readmission to hospital. The service enables individuals to regain confidence, motivation and skills to self-manage, and encourages them to engage with the local community to improve socialisation and wellbeing.
