The Integrated Public Health Nursing Service will provide leadership, co-ordination and delivery of the 0-19 Healthy Child Programme (0-19 HCP) in Sefton. An essential part of the children's services ‘system’, it will make a major contribution to our Children and Young People's Plan ambitions by— Improving health and wellbeing outcomes for children and young people,— Identifying additional needs early,— Building resilience, and— Reducing health inequalities by providing effective universal and targeted interventions for children and their parents.The 0 — 19 HCP will be delivered to residents living in Sefton. All families with a child aged 0 — 5 years and all pregnant women currently resident in the local authority area must be proactively offered the HCP.
