The service will deliver personalised support that promotes independent living to adults with a learning disability living in their own home.The Service will be provided for individuals who meet the Purchaser's eligibility criteria 'Fair access to Care' as agreed and amended from time to time.The Provider will deliver the service to the individual in accordance with the Purchasers assessment, support plan and outcomes. In addition, the Provider will develop a person centred support plan that forms the basis of their support to the individual and describes how the agreed outcomes are to be delivered. This will be written in consultation with the individual, family members and advocates. The individual's support plan will form the basis for evaluating the level of quality delivered for this service.To deliver supported living to individuals within their own home that offers an enabling environment. To deliver this through giving people more opportunities to progress with a view to becoming less dependent on support in the future and the opportunity to consider options to move on such as an individual tenancy with minimal or floating support, an individual tenancy with support on-site or independent tenancy.To provide a good quality, personalised outcome focused service appropriate to the needs identified in an individual's Support plan and to demonstrate this through assistance with personal, practical and social/emotional tasks associated with ordinary living and a fulfilling and meaningful life.The service will support positive risk taking in individuals' daily lives that is personally meaningful to people who use services and promotes an individual's assets, wishes and aspirations.The service will be available 356 days per year with 24 hour support delivered as required where this is identified. The Purchaser is looking to develop a Service that moves away from 24 hour provision and considers alternative overnight support such as assistive technology, on call and floating outreach support. Where overnight support is identified as a requirement through waking night or sleep-in provision, the latter may vary and will be up to a maximum of 10 hours per night.The Provider will work to an overall cost for the delivery of the service to support individuals needs and outcomes. The Provider will manage any change to an individual's needs and outcomes within these costs.To work in conjunction with Housing Providers and the individual to ensure tenancies are maintained.
