NHS Heywood, Middleton and Rochdale Clinical Commissioning Group (NHS HMR CCG) are looking for suitably qualified Providers to submit tenders for The Provision of a Primary Care Mental Health Pathway.The aim of the Provision of a Primary Care Mental Health Pathway is to offer a seamless service working in a partnership approach to deliver a cohesive pathway incorporating, primary, secondary and third sector health care providers. The Primary Care Mental Health Pathway will establish effective working partnerships and networks with the wider range of social and health interventions commissioned to support the health and well-being of the Heywood, Middleton and Rochdale Population. The Provision of a Primary Care Mental Health Pathway will be made up of the 2 following Services of which Providers will be required to provide:— Primary Care Psychological Therapies that must be accessible for 16+; and— The Getting Help Mental Health offer that must be accessible for 18+.
