Wrexham County Borough Council would like to invite Expression of Interest from suitably qualified and experienced service providers to provide domiciliary care and housing support services for vulnerable adults over 55 at Plas Telford, the Council’s Extra Care Scheme at Acrefair, Wrexham, LL14 3AU.The site has in total 54 apartments: 42 with two bedrooms and 12 having one bedroom. Half of the apartments will be occupied by tenants who have no personal support needs. Each tenant's care package will be based on an individual assessment of need.Applicants must be registered with the Commission for Social Services in Wales (CSSIW), and be able to provide domiciliary care services and housing related support services that will enable Service Users to sustain residency within the Extra Care Housing Scheme, whilst preserving maximum independence, minimising risks and promoting quality of life.The contract is intended to run for 2 years with the possibility of a 2 year extension in yearly increments.The closing date for submission of Expressions of Interest, which must be made via the Sell2Wales website, will be 12 noon on Tuesday 28 June 2016NOTE: To register your interest in this notice and obtain any additional information please visit the Sell2Wales Web Site at http://www.sell2wales.gov.wales/Search/Search_Switch.aspx?ID=47277.
