This contract is for the provision of flexible support for carers within Flintshire and covers the following aspects:-Promote greater awareness & support to Young Carers from all agencies eg schools and leisure services.Support to Carers to access Learning, Leisure and Work.Family Support Service for Carers of people with Mental Health problems and Substance Misuse problems.Training for Carers and Young Carers.Planned and regular breaks for Carers targeted at specific and generic needsEquitable, flexible and responsive respite servicesCarers and Young Carers Emergency Card.Carers /family liaison service for carers caring for an adult with Mental Illness Carer , Parent Carer and Young Carer support networks.Carers Grant Scheme.IMPORTANT:Only those providers with a proven track record of contracts of this kind are invited to bid for this work.For more information, please refer to the attached document entitled Carers Outcomes - Action Plan.
