Descriptions
Stockport Council Adult Social Care is embarking on an exciting and ambitious development bringing the former St Thomas’ hospital site back to life and transforming the way that we promote independence and provide the best possible outcomes for people. The St Thomas’ Centre for Living Well will be a new facility that pushes the boundaries of design and innovation to create modern home-like accommodation where people are able to regain their independence. It will be located alongside a new residential community developed on a site steeped in rich heritage, with refurbished listed buildings and new homes built to the highest levels of sustainability creating an affordable place to live that will attract a truly inter-generational community with all the benefits of Town Centre living. It is due to open in early 2027. The Centre of Living Well will provide locally based, high impact services with a focus on: • step-up facility to prevent hospital admissions • step-down facility to accelerate hospital discharge through reablement • access for respite or short breaks • En-suite, studio and bariatric accommodation within 5 households across 3 floors, totalling 82 intermediate care, including supporting people with complex care needs and a diagnosis of dementia • A state of the art technology lab, training facilities, gymnasium and outdoor podium garden • Ground floor facilities for the community It is our ambition for the Centre to be a Beacon of Excellence for intermediate care and we aim to achieve considerable outcomes for the people of Stockport as part of our Live Well strategy, as well as greater efficiencies, and market sustainability. We can achieve this ambition by working together with health and care partners across Stockport to provide the best intermediate care within one location, with both clinical and reablement expertise available onsite and with access to the facility from both the community and from hospital. After recently undertaking some initial market engagement to test the level of interest from Residential and Nursing Care Providers to operate St Thomas’ as CQC Registered Providers, we are planning for the procurement of the future Operator of St Thomas’ and are keen to involve interested Residential and Nursing Care Providers in developing the service specification required for the procurement process. The Council believes that direct input into the development of this specification from Care Providers would be highly beneficial both to the success of the procurement process and to those Providers who are interested in submitting a tender. The council is holding an In-person Workshop on 9th December at 12.30pm at the Council Offices in Stockport, to gauge views and input from Providers on important aspects of the service specification, such as; • The Operating Model and how the 82 beds will be used to support the demand for intermediate care bed-based provision and their category of needs. • The staffing model; roles, responsibilities and staffing numbers for operating the facility safely and affordably. • Timeline requirements for mobilisation. • Partnership working practice with those in reaching into St Thomas’ such as Therapists, and multi-disciplinary teams of Social Workers and GP’s, etc • The Assistive Technology that can improve the experience of the individual as well as the workforce. • How managing the facilities and amenities can create a great residential and working environment. If you are interested in attending this Workshop to influence these aspects of the service specification, please contact Michelle Moseley, Programme Manager, email: michelle.moseley@stockport.gov.uk to book your place.
