Funding provided in this Contract is for the provision of a minimum of 1.4 Whole Time Equivalent Third Sector Brokers, to be based within a multi disciplinary intermediate care service providing a seven day service across the Wrexham County Borough including the Wrexham Maelor Hospital and will be managed by Wrexham County Borough Council Adult Social Care department.The third sector broker post is a new post within the intermediate care service and forms part of a eight month project to enhance the size and scope of the intermediate care service across Wrexham to support older people to remain as independent as possible within their own homes and communities, avoiding unnecessary admission to hospital and supporting timely discharge home where a hospital admission has been necessary.NOTE: The authority is using eTenderwales to carry out this procurement process.
