The primary aim of the Tier 3 Specialist Adult Weight Management Service aims to achieve the best health and well-being outcomes for individuals referred to the service by:— Improving the health outcomes for individuals who are overweight or obese and who often have significant co-morbidities over a 12 to 24 month period;— Motivate, empower and facilitate individuals to make sustainable healthy lifestyle changes, which result in weight loss, increase in self-confidence and self-efficacy.— Appropriate holistic multi-disciplinary assessment with a menu of healthy lifestyle interventions (health eating, physical activity and behaviour change) as part of the Service offer.NHS Crawley and NHS Horsham and Mid Sussex invites applications from suitably qualified providers who feel that they can deliver requirement contained in the Service Specification to manage and deliver a Tier 3 Specialist Adult Weight Management Service that is evidence based for the management of adults (18 years and over) who are severely obese (BMI 40+) or obese with severe co- morbidities (BMI 35+).Tier 3 services form one part of the weight management pathway and are designed to treat obesity and morbid obesity in a specialised multi-disciplinary community setting with an emphasis on psychological support for the increasing numbers of people who have failed to achieve a healthy weight after accessing Tier 2 services. Tier 3 services also provide support for those patients who are going forward to surgical intervention services, Tier 4 and for those who are two years post surgery.Aims of the Service— To provide a medical management of obesity service for severely obese patients;— To provide an alternative to surgical intervention;— To prepare patients adequately if a surgical intervention is required;— To provide a maintenance plan for patients who are exiting the service;— To provide a Crawley CCG and Horsham and Mid Sussex CCGs - wide service for patients who meet the referral criteria;— To improve health outcomes in severely obese patients;— To reduce the health burden of obesity;— Motivate, empower and facilitate individuals to make sustainable healthy lifestyle changes, which result in weight loss, increase in self-confidence and self-efficacy.
