Coastal West Sussex CCG wish to commission Tier 3 Weight Management services for adults patients across its geographic area, following on from pilot services which have been running across the area since 2010.The aims of the service are:— To provide a medical management of obesity service for severely obese patients;— To provide an alternative to surgical intervention;— To prepare patients adequately if a surgical intervention is required;— To provide a maintenance plan for patients who are exiting the service;— To provide a CCG- wide service for patients who meet the referral criteria;— To improve health outcomes in severely obese patients;— To reduce the health burden of obesity;— To provide a service to patients referred as part of the MSK knee and hip surgery pathway, to reduce weight / BMI.The service will provide assessment and treatment of patients led by a multi-disciplinary team (MDT) approach to include bariatric physician, CBT, dietician, nurse and exercise specialist with access to clinical support to address complex issues.Coastal West Sussex CCG are seeking a single provider able to offer services across the full geographic area of the CCG and to link into the full weight management pathway to ensure patients are managed and referred as necessary to other partners in the pathway; namely tier 2 and tier 4.
