Virtual Ward Model

We are looking to establish a virtual ward model at KGH (KGHCARE@Home) with a baseline bed capacity of 20 with flexibility to increase to 28 beds, possibly to 33.Clinical pathways will include:— IV antibiotic provision, including osteomyelitis, cellulitis, joint infection, pneumonia, subacute bacterial endocarditis, urinary tract infection,— Non-weight-bearing patients,— Frail elderly – therapy, nursing, personal care,— Post-fall rehabilitation e.g. no fracture or fractures for medical management only,— Total knee joint replacements,— Wound care – VAC/larvae,— Bridging social care delays – Discharge to assess, delays for residential homes, delays for nursing homes,— Bridge package of care for stroke patients whilst community stroke team provides therapy,— Parenteral fluids and electrolyte replacement/monitoring,— Parentral Management of patients with hyperemesis,— Stage 1 acute kidney injury management.The service must offer 24hr healthcare professional support to patients.
