The NHS Wales Shared Services Partnership, hosted by Velindre NHS Trust are acting on behalf of Cardiff and Vale University Local Health Board who are seeking to establish a contract for the provision of a Volunteer Sector Support for Administration Admission Avoidance / Timely Hospital Discharge.In line with its ‘Home First’ philosophy, the Health Board remains committed to providing community based services by which to support older people to regain and maintain their independence in the community. This tender seeks support for a model of service that will interface at a number of key points within the acute care pathway, these will be;1) Prior to admission, the service will work directly with the Cardiff Community Resource Team to support individuals who are failing and, without rapid support, are at risk of admission.2) Within the Emergency Unit at UHW, the service will offer support for patients who do not require admission, but who require advice whilst in the department or, post discharge from the unit, to ensure that they have information to enable them to seek community based advice and support to maintain them at home.3) On discharge, the service will work with wards and the Cardiff Community Resource Team to support the timely discharge of patients who require time limited support at home to enable them to regain independenceThe service will offer a range of interventions depending on patient identified needs and the request for support as identified by the referrer. Interventions may include:-- Telephone advice- Visiting the client on an intermittent basis over the course of a 2 week period, where there is a need to ‘resettle’ a client post hospital discharge- Contributing the delivery of a care package, for a defined period (not usually longer than 10 working days) as prescribed by the Community Resource TeamNOTE: The authority is using eTenderwales to carry out this procurement process.
